Registration Procedure

REGISTRATION IS BY APPOINTMENT ONLY

• In order to register a  new student, the following information is required from the transferring school:

• Transfer Card

• Health History

• Report Card

• Child Study Team records if applicable

• Mount Ephraim Public Schools requires the guardian to provide copies of the following:

• Birth Certificate

• Legal/Custody Papers (if applicable)

• Immunization Record (Kindergarten Registration only)

• Proof of Residency - two forms (example:  utility bill, tax bill, etc.)

• Guardian must fill out the following forms which will be provided by the Mount Ephraim Public Schools main office:

• Registration Form

• Request for Records

• Misc. Notes:

• The first day of school for next year will be _________________.

• Kindergarten students must be 5 years old by October 1.

• Kindergarten schedules for the first 3 days of school will be forwarded over the summer.
